UnitedHealth Beats and Raises Again, Anchors 2027 Growth, but Commercial Remains the Weak Link

UnitedHealth Group delivered another beat-and-raise quarter that strengthens the margin-recovery story. Adjusted earnings of $4.69 per share topped the $4.60 Earnings Whisper number, a 2.0% surprise, and cleared the $4.12 consensus estimate by a wide margin. Earnings grew 60.6% from $2.92 a year ago. Revenue of $112.69 billion beat the $111.38 billion consensus by 1.2% but fell 0.4% year over year. Management raised full-year adjusted EPS guidance to at least $20.50 from $19.50-$20.00. That is the third consecutive raise from an initial outlook of more than $17.75. The central tension is the source of the growth. UnitedHealth is expanding profit on a shrinking book of business, and its largest commercial franchise is still moving the wrong way.

The quality of the beat deserves some scrutiny. The medical care ratio improved to 88.4% from 89.9%, but that figure includes $610 million of favorable prior-period reserve development tied to 2026 dates of service. GAAP EPS was $4.56. Adjusted operating earnings exclude $224 million of net divestiture gains and a $95 million release of loss contract reserves. Even so, the improvement looks real across segments:

• The operating cost ratio fell to 12.8% from 13.5%.

• UnitedHealthcare's operating margin rose to 3.3% from 2.1%.

• Optum Health earned $735 million versus just $41 million a year ago.

• Operating cash flow was 1.2 times net income.

Sequentially, earnings from operations fell to $6.6 billion from $8.0 billion in the second quarter, which reflects the normal back-half seasonality of the risk business.

The flat top line is a deliberate result of repricing and exiting unprofitable business. UnitedHealthcare served 48.3 million medical members, down from 50.1 million a year earlier. Medicare Advantage and related complex populations have contracted by 935,000 since year-end. Medicaid membership continues to fall with eligibility changes, and Optum Health revenue dropped 10% as the company left thin value-based markets. The call showed management steadily lifting its segment targets:

• Medicare Advantage margins are now expected near the top of the 2-4% range, up from "above 3%" last quarter.

• Medicaid is expected to finish moderately better than -1%, with rated margins targeted by the end of 2028.

• Optum Health reiterated its path to 6-8% margins by 2028.

The tone has clearly moved from turnaround defense to offense. Management called 2027 EPS growth at the low end of its 13-16% long-term range a "reasonable starting point" and suggested consensus may be too low.

The catalysts are concrete. UnitedHealth has expanded its Medicare Advantage footprint to more than 94% of eligibles, with a focus on special-needs plans and HMOs, and expects roughly 75% of members in 4+ star plans. Optum Rx's external membership now exceeds 50%, with retention in the high 90s. Optum Insight's Integrity One has grown from 2 clients to 15 live and 24 implementing. Bears still have legitimate ammunition, however:

• Commercial cost trend remains north of 11%.

• The commercial margin recovery has slipped for a third straight quarter and is now "stretched out" past 2027.

• No Surprises Act arbitration awards are approaching about 6% of medical spend in some regions.

• Accelerated 2027 investment in UnitedHealthcare and Optum Insight will be a headwind.

• Optum Health's guidance implies a meaningfully negative fourth quarter.

• Medicaid reforms under the OBBBA phase in through 2030.

• Debt-to-capital rose to 41.7% from 41.2%.

The market setup is more complicated than the surprise implies. Investor sentiment fell from essentially neutral (0.0085) to -0.2463, a meaningful shift into negative territory heading into the report. Shares were $377.76 on the report date, down 15.9% from the $449.39 open after the July report. They have given back most of the post-report spike to a 52-week high of $461.62 on July 16 and now sit 18.2% below that level. The stock touched an inter-earnings low of $362.60 on October 1. It remains just 4.2% above that low and 5.6% above its 200-day moving average of $357.89. The Earnings Whispers price trend is neutral, while the AVWAP trend remains negative, consistent with a stock that has been sold despite rising guidance.

The bottom line is that UnitedHealth's earnings recovery is progressing faster than its revenue base or its stock price suggests. Medicare Advantage, Medicaid, Optum Health and Optum Insight all moved forward, and management gave its first explicit 2027 framework. Commercial remains the clear laggard, and favorable reserve development flattered the quarter. A skeptical market will likely need to see that 13% floor hold up against rising investment spending and stubborn commercial trend before it rewards the guidance raise.
